w/o 21/11/1972. The Naval tail shaft break joint disconnected causing loss of tail rotor thrust. It descended, yawing and struck the ground at Yeovil, Somerset and rolled onto its port side. It was helping ice patrol ship HMS Endurance and research vessel Ernest Shackleton when it came down on the Weddell Sea coast. The aircraft was recovered and transported to the Accident Investigation Unit at Yeovilton. Remains FSAIU

w/o 01May89. Crashed into the bush at Vipingo, eighteen miles from Mombasa, Kenya and exploded. It was en-route from the ship to Mombasa when the port cabin door was opened. Two ball bearings were missing from the top rail causing the door to detach. It struck the tail rotor causing the tail rotor shaft to shear off, the tail rotor pylon then broke off.  All eleven on board were killed.

w/o 08Dec04. The helicopter had been scrambled after two sailors on the frigate HMS Montrose said they had heard cries for help coming from the water off the Cornish Coast. During the search for the ‘men overboard’, contact with the aircraft was lost. A search commenced for the four crew. Mangled parts of the helicopter were found in water 85 metres deep, 19 miles off Lizard Point, by an unmanned submarine. The four crew were killed. The aircraft was recovered and transported to the Accident Investigation Unit at Yeovilton. Remains FSAIU.

w/o 18Feb97. Broke free from its lashings while preparing for take off from HMS Monmouth in rough seas fifteen miles south of Eddystone rocks on the south Devon coast. It fell over the side of the ship and sank. Both crew were rescued uninjured by the ships seaboat. It was recovered and taken to Fleetlands and was deemed to be damaged beyond repair. - Tobe Yeovilton  Gate Guard in 2006 (AW326).

w/o 27Oct01. Crashed into the Arabian Sea after an Omani warship collided with the aircraft as it was hovering above the water. The pilot and observer had seconds to abandon their cockpit as it filled with water and rapidly sank. The men, based on HMS Marlborough, suffered minor injuries and were taken to hospital onboard the aircraft carrier HMS Illustrious. The incident happened during exercise ‘Saif Sareea’ (Swift Sword) a joint exercise between British and Omani forces. The Lynx was dropping a dummy torpedo into the sea when an Omani vessel sailed into it from behind, catching the helicopter's rotor blades on its rigging. The wreckage arrived back at Yeovilton on 3rd December. It was finally dumped for spares use during August 2007. (Desert Storm as 342 Brilliant Flt.)

CAT.3/4. 10Sep2009-Collided with high-tension cables while on a low-flying training exercise near  Lynton, north Devon, and force landed at about 1350hrs in a field on Exmoor, between Lynmouth and Foreland Point. The 2 crew were unhurt and damaged appeared to be confined to a smashed windscreen and some rotor blade damage. It was active again by late November.
